I rarely had to work with competing services locally before the AI era (I needed much less context-switching), but now it's pretty much essential, with tens of worktrees and x10 services as well. What do you think is the best solution for all of this to work together? For example, I have a db. You say let's dump it in the mix, so every copy of the codebase accesses the same db. What happens when multiple agents try to hammer the same table, rows, etc?

I am not sure what you mean, the agents to perform changes on the database must use a migration. The database servers are shared, same container, but each app has its own database. Regarding reads and writes, should not be an issue, unless you are causing some race…
